Health Supervision Program
Medical oversight of your overall health and well-being
Unique to our agency is a program to provide monitoring and care services for those who have moved beyond the acute stages of an illness, but who have a need for continued health assessments. The periodic nursing, physical therapy, and social work visits provided through our Health Supervision Program offer ongoing support, preventive care measures, the opportunity for early detection of medical issues, and teaching of positive health practices. Our staff will communicate with your physicians to keep them informed of progress or concerns. Thanks to town funding and donations, Health Supervision visits are available at no cost.
