Since 2012, the Farmington Valley VNA has been distinguished among the top 25 percent of Medicare-certified agencies in the nation. This "HomeCare Elite" ranking is developed by ABILITY® Network, a leading information technology company helping providers and payers simplify the administrative and clinical complexities of healthcare; and sponsored by DecisionHealth, publisher of: Home Health Line, The Complete Home Health ICD-10-CM Diagnosis Coding Manual and The Home Health Coding Center.
This process rates the performance of home health care agencies using publicly available data in the areas of quality care/patient outcomes, best practice implementation, patient experience, quality improvement and consistency, and financial performance measures. Attaining a 6th consecutive "HomeCare Elite" designation reflects our steadfast commitment to providing high quality, comprehensive in-home health care, community-based illness prevention programs, and wellness initiatives.

Home Health Quality of Patient Care Star Rating
The Centers for Medicare & Medicaid Services (CMS) have established a "star rating" system for ranking home health agencies. Star ratings summarize data to help agencies identify areas for improvement and to help consumers make educated decisions when seeking a health care provider.
The Home Health Quality of Patient Care Star Rating is calculated using the process and outcome measures reported on the “Home Health Compare” website. Agencies are measured on timely initiation of care, drug education regarding medications, current flu vaccines received, improvements in ambulation, transferring, bathing, pain interfering with activity, shortness of breath, and re- hospitalizations. A rating of 4 or 5 stars means the agency performed better than most other agencies on selected measures. The Farmington Valley VNA receives consistent ratings as a 4-Star Medicare certified agency.

CAHPS® Home Health Care Survey
The Consumer Assessment of Healthcare Providers and Systems (CAHPS) provides details about patient experiences and the quality of care received through Medicare-certified home health care agencies. Consumers can compare ratings of up to three agencies side-by-side on the www.medicare.gov/homehealthcompare website.
The Farmington Valley VNA consistently remain above the state and national averages for overall satisfaction with care, likelihood of a patient recommending our agency, and the care we provide to our patients.
