We believe that delivering palliative care for those in life's final stages is only one facet of our duties as hospice caregivers. Providing support, maximum comfort and encouragement to our patients as well as their loved ones is crucial to our role as compassionate caregivers. Patient care focuses on maximum comfort, minimized pain, and facilitating supportive relationships between patients and their loved ones.
We understand that no two situations are alike, so our Hospice Interdisciplinary Team coordinates a plan of care designed for every unique case. Our Medical experts, bereavement counselors and caregivers collaborate with patients, their physicians and their loved ones to support the dignity, the hope and the health of our hospice patients.
Our Hospice services include:
A local, hospice employed physician who provides certification and oversight of the hospice plan of care, consultation to hospice team members regarding medical care of the patient, and occasional home visits as needed.
Registered nurses who are certified and experienced in hospice/palliative care provide compassionate, specialized nursing to hospice patients and support to family members. Services are available seven days per week, twenty-four hours per day and 365 days per year.
Certified home health aides provide assistance with bathing, dressing, feeding and other personal care to maintain patients' comfort and dignity.
Registered dieticians provide teaching and guidance regarding special dietary needs of the hospice patient.
Registered physical therapists and licensed occupational therapists provide specialized therapy to hospice patients in order to help them maintain flexibility, muscle strength and mobility.
Certified and licensed speech/language therapists provide assistance with speech impairment and swallowing difficulty as indicated.
Working arrangements are in place with a number of local pharmacies to provide needed medications, even in an emergency. A registered pharmacist is an integral member of the hospice team who provides consultation and guidance regarding medication management, side effects and pain control.
A local, hospice-employed clergy with hospice training and experience provides pastoral counseling and spiritual support to hospice patients and their caregivers in the patient's home. Guidance and support is given to hospice team members regarding the spiritual needs of hospice patients and their families.
Compassionate volunteers with specialized training provide support, friendship and companionship to hospice patients. Volunteers also offer respite to family members by remaining with the hospice patient to allow family members to leave the home for periods of time.
A licensed medical social worker provides bereavement and support services to assist with grief and coping upon the patient's death. Home visit and telephone follow-up, and bereavement support groups are provided as indicated.
